Privacy and Confidentiality Statement
At PSYTHERA we prioritize the protection and confidentiality of your personal information. We handle your data in compliance with the General Data Protection Regulation (GDPR) effective from 25 May 2018, ensuring meticulous care in processing and storing your personal data.
Record Keeping and Confidentiality
As your therapist, I am required by law under the Medical Treatment Agreement Act (WGBO/Wkkgz) to maintain a confidential record of our sessions. This includes essential details such as your full name, date of birth, contact information, session dates and times, therapy goals, summaries of our conversations, and relevant medical and insurance information (if applicable). This information is securely stored in both physical and electronic formats, including my appointment booking and invoicing systems.
Data Security and Access
Your personal data is kept secure and confidential. You have the right to access your file at any time and request corrections or deletion, as permitted by Dutch legislation. Unless specified otherwise, files are retained for a minimum of 20 years to meet legal requirements.
Supervision and Professional Standards
As part of maintaining high standards of care, I participate in supervision where I discuss aspects of our work. Your identity and the content of our sessions remain strictly confidential during these discussions.
Video and Voice Recording Policy
Recording of sessions, whether video or voice, is prohibited without explicit written consent from all participants.
Confidentiality and Incident Reporting
Your therapy files are accessible only to me, and the content of our sessions remains confidential except where required by law or in cases of imminent harm. I will strive to consult with you before disclosing any confidential information to third parties.
Complaints Procedure and Incident Reporting
In compliance with Wkkgz regulations, healthcare providers are obligated to report incidents and maintain a complaints procedure. If necessary, private information related to our sessions may be shared with relevant bodies to address incidents or complaints.

Cancellation policy
If you are unable to come to your scheduled appointment, you must notify us at least 24 hours in advance. If you do not show up or if you cancel less than 24 hours in advance, you will be charged the fee of the appointment.
